This video was recorded from the January 31st, 2011 training, presented by Anna Gruver and lasts 46 minutes. To receive your ECChange ID password, you must watch the Confidentiality Video, successfully complete the short F5AC Confidentiality Quiz and sign the F5AC Confidentiality Oath. Then, send an email to Virve Hernandez at Virve.Hernandez@first5alameda.org letting her know that you have completed both items and are mailing your signed Oath. Virve will begin the ECChange ID process and you will be issued a password. Then, you must mail your signed oath and completed quiz to :

Confidentiality Frequently Asked Questions - Aug 2011 This document is a compilation of all questions with their responses raised at our trainings, teleconferences, and meetings to date (9.13.2011). It is important to note that these are the perspectives and guidelines of First 5 Alameda County. We understand if your agency’s policies and procedures may differ.
Caregivers Authorizations Affidavit (CAF) This form should be used in conjunction with First 5 Alameda County authorizations. It is given to those adults who live with and are the primary caretakers of the child. Also, the CAF is used for those clients whose legal guardian or parent is not physically available to sign authorizations because they do not live with the client and the provider does not have access to the parent or guardian for authorization of services (i.e. parent is homeless, lives in another state or country, is deceased).
